INFANT FORMULA PREP FORM This is a permission form to allow us, the teachers of chosen childcare, to prepare your infant's formula. The parent/guardian's responsibility to provide their infant's formula unless the infant runs out of formula during the day. It is the parent/guardian's choice to accept the formula provided by the daycare. The formula that we are currently using is GERBER GENTLE or SOOTHE.
